If you intend to consent the adoption and termination of parental rights you shall respond to the Petition within twenty (20) days from the date you receive this Notice and the attached Petition. FAILURE TO RESPOND SHALL BE TREATED AS A DEFAULT AND YOUR CONSENT TO THE ADOPTION SHALL NOT BE REQUIRED WHICH MAY RESULT IN TERMINATION OF YOUR PARENTAL RIGHTS.
