Petitioner.
PETITION TO ESTABLISH PATERNITY
COMES NOW the Petitioner and states:
1. Petitioner is a resident of Bernalillo County, New Mexico, thus giving rise to jurisdiction.
2. That he was born in Phoenix, Arizona County of Maricopa on 9 /6/1990. See Exhibit 1, attached.
3. That he is the natural son of Liliana Mantilla and Roy Madrid.
4. That his birth certificate clearly shows the name of his mother. See Exhibit 1, attached.
5. That, through an oversight, the name of his father is not shown on his birth certificate. See Exhibit 1, attached.
6. That his mother has executed an affidavit as to the identity of his father. See Exhibit 2, attached.
7. That his father has executed an affidavit acknowledging paternity. See Exhibit 3, attached.
8. That it would relieve him of a multitude of anxieties and concerns to have his father acknowledged as such on his birth certificate.
WHEREFORE, the Petitioner prays that:
A. The Court enter an Order finding Roy Madrid, the elder, to be his father.
B. Grant the Petitioner such other relief that it deems fit and proper.
