COME NOW the Petitioner’s Father and Mother, BOBBY JAY ANDERSON, and TIFFANY RAE VIGIL, as Next Best Friends to JOSHUA JAY VIGIL, and requests the Court to change their Son’s name from, JOSHUA JAY VIGIL to JOSHUA JAY ANDERSON.
1. That JOSHUA JAY VIGIL was born on July 5, 2012.
2. That JOSHUA JAY VIGIL is a resident of the State of New Mexico and he has lived in New Mexico since he was born.
3. The Court has jurisdiction over this matter.
4. The Petitioner’s Social Security Number is: 097-95-0456
5. The Petitioner is not filing this petition to avoid legal process.
WHEREFORE, the Petitioner’s Father and Mother, BOBBY JAY ANDERSON, and TIFFANY RAE VIGIL, pray the Court for an Order changing their Son’s name from JOSHUA JAY VIGIL to JOSHUA JAY ANDERSON.
