TO: PATIENT OF CENTER FOR REPRODUCTION MEDICINE OF NEW MEXICO, P.A.
NOTICE IS HEREBY GIVEN that the Center for Reproductive Medicine of New Mexico, P.A. (“CRM”) (201 Cedar Street, Suite S1-20, Albuquerque, New Mexico, 87106/ (505) 247-3333 or (866) 926-8483) will close effective July 1, 2016. After July 1, 2016, Drs. Thompson and Caperton will form separate medical practices in order to pursue their individual practice objectives.
Patients of CRM are advised that their medical records and cryopreserved materials, if any, will be located at the CRM office until July 1, 2016. After July 1, 2016, medical records and cryopreserved materials, if any, will be located at the new office of the treating physician. Patient records and cryopreserved materials of Dr. Caperton’s patients will be located at his new practice, Caperton Fertility Institute, LLC, at 6500 Jefferson NE, Suite 250, Albuquerque, New Mexico. Dr. Caperton may be reached by phone at Caperton Fertility Institute, LLC at (505) 702-8020. Patient records and cryopreserved materials of Dr. Thompson’s patients will be located at his new practice, The Fertility Center of New Mexico, LLC, at 201 Cedar SE, Suite S1-20, Albuquerque, New Mexico. Any patients of CRM who wish to direct their records and/or materials to a different provider may contact Drs. Thompson and Caperton at CRM through July 1, 2016, and at their separate practices following July 1, 2016.
