COMES NOW, Yeavette Evangeline Andujo, by and through her attorneys of record,
JUSTICE LEGAL GROUP (Amanda D. Navarro/Anthony Griego), and hereby states as follows:
1. This Court has jurisdiction of the party and subject matter herein.
2. Venue is proper with this Court.
3. Petitioner is Yeavette Evangeline Andujo, born on November 4, 1960.
4. Petitioner is a resident of the State of New Mexico and has been said resident for at least six months.
5. Petitioner resides in Bernalillo County, New Mexico.
6. Petitioner seeks to change her name from Yeavette Evangeline Andujo to Yvette Evangeline Baca. Petitioner is over the age of fourteen.
7. There is no reason to the contrary that Petitioner’s name not be changed to Yvette Evangeline Baca pursuant to NMSA 1978 §40-8-1.
WHEREFORE, Petitioner requests this Court to change her name from Yeavette Evangeline Andujo to Yvette Evangeline Baca and for such other relief as this Court deems proper.
