NOTICE IS HEREBY GIVEN that Cathy M, Chavez, resident of the City of Albuquerque, County of Bernalillo, State of New Mexico, and over the age of fourteen (14) years old, date of birth May 7, 1959 has filed a Petition to Change Name in the Second Judicial District Court, Bernalillo County, State of New Mexico, wherein she seeks to change her names as follows:
1. Petitioner requests that her birth name on her birth certificate with the New Mexico Department of Vital Statistics from Mary Ann Katherine Garley to Cathy Mary Ann Garley.
2. Petitioner requests that her current legal name remain Cathy Mary Ann Chavez to reflect her married status.
THIS MATTER WILL BE HEARD before the Honorable DENISE BARELA SHEPERD, District Court Judge on the 1st day of November 2017 at the hour of 10:30 a.m. at the Bernalillo County Courthouse 400 Lomas Blvd N.W., Albuquerque, New Mexico 87102.
