NMSA 1978, and will submit his application to the Court before the Honorable Nancy J. Franchini, Division V District Judge, 2nd Judicial District, Bernalillo County Court House, 5th Floor, 400 Lomas, N.W., Albuquerque, NM 87102 on the 28th day of March, 2018, at 1:30 p.m. asking that his name be changed from Juan Ramon Perez to Ray J. Perez.
Petitioner resides in Albuquerque, County of Bernalillo, State of New Mexico.
