PETITION FOR CHANGE OF NAME
COMES NOW the Petitioner, and Petitions the Court to enter an Order pursuant to §40-8-1 NMSA (1978) to change her name. As grounds the Petitioner states:
1. That she has been a resident of Bernalillo County, New Mexico for more than six months.
2. That this Court has jurisdiction over this Matter pursuant to §40-8-1 NMSA (1978) and venue is proper in the Bernalillo County District Court.
3. That she has attained the age of majority. See Exhibit 1.
4. That she is the natural daughter of Robert O. Hickey. See Exhibit 1.
5. That she is the natural daughter of Evelyn V. Beriso. See Exhibit 1.
6. That to honor her mother, she would like to bear her mother’s name in addition to that of her father and change her name to Willow Victoria Beriso Hickey.
WHEREFORE, the Petitioner prays the Court will grant this Petition changing her name as stated above.
