CHANGE OF NAME PETITIONER, by and through the undersigned legal counsel, for his Petition for a Change of Name, states as follows: 1. This Petition is brought pursuant to 40-8-1, et seq., NMSA (2010). 2. On July 21st, 2000, A petition was filed seeking the same relief herein, was filed in Bernalillo County, New Mexico with Cause No. D-202-CV-7352. 3. That matter was dismissed for Lack of Prosecution on August 17th, 2001. 4. Petitioner’s current name is STANLEY GERALD POTTER. 5. Petitioner’s date of birth is January 27th, 1968. 6. Petitioner’s New Mexico Driver’s License reflects the name JERRY R. POTTER. 7. Petitioner’s current mailing address is 156 Rincon Loop, Tijeras, New Mexico. 8. Petitioner is not seeking to defraud creditors by changing his name. 9. Petitioner is not a sex offender who is required to register under any state of federal law. 10. Petitioner, has simultaneously with this Application, submitted for publication in the Health City Sun, a newspaper of weekly circulation in this Jurisdiction, notice of his application as required by 40-8-1 NMSA (2010). 11. Petitioner wishes to change his name to comport with his Certificate of Birth. 12. Petitioner wishes to change his name to JERRY RAY POTTER. WHEREFORE petitioner respectfully requests that this Court grant his Petition for Name Change, and for other and further relief the Court deem proper in the premises.
