PETITION TO CHANGE NAME
Comes now the Petitioner, Aida M. Davalos, whose date of birth of January 16, 1946, and who is represented by Barbara J. Koenig, Attorney at Law, pursuant to §40-8-1 NMSA 1978, states as follows:
1. Petitioner, Aida M. Davalos, is a resident of Albuquerque, Bernalillo County, New Mexico.
2. Petitioner is over the age of fourteen (14) years of age.
3. Petitioner wishes to change her name from Aida M. Davalos to Maria Aida Candelaria.
4. There is no reason to the contrary why Petitioner’s name should not be changed to and established as Maria Aida Candelaria.
WHEREFORE, Petitioner prays
1. That the Court set a hearing in this matter; and
2. Following the time set for the hearing, the Court enter its Order changing and establishing Petitioner’s names as Maria Aida Candelaria.
