You are hereby notified that suit has been filed and is now pending against you in the District Court of Bernalillo County, New Mexico. In said suit, David Romero, as the Personal Representative of the Estate of Monica Gonzales, is the plaintiff and you are a defendant. The general object of said suit is compensation for your medical negligence in the care of Monica Gonzales and that medical negligence is alleged to have been a cause of Ms. Gonzales’s death. Please be further advised unless you appear, answer, or otherwise file a pleading prior to your time to respond expires, Plaintiff will take judgment by default against you for the relief prayed for in the complaint.
