You are hereby notified that the above-named Plaintiff has filed a civil action against you in the above-entitled Court and cause, the general object thereof to obtain judgment against you for personal injuries and other claims for violation of her rights.
That unless you file a responsive pleading or motion in said cause within twenty days of the last publication of this notice, judgment by default will be entered against you.
