This is a minor who hasn’t contacted legal guardian about name change.
This minor is unable to make this decision on her own. She has not consulted guardians, and is failing most of her classes as well as ditching at least 3 times per week. The adults she is staying with do not provide proper supervision for her, and approve of her poor behavior and poor academics.
